For 1 Child:
-$20 per week @ $80 per month total
For Multiple Children:
-2 Children: $15 per week each child @ $120 per month total
-3 Children: $15 per week each child @ $150 per month total

INSTRUCTIONS:
1. Fill out the Registration Form/Release below and "Submit".
2. Make Payment through PayPal (See Instructions below).
PAYPAL PAYMENT INSTRUCTIONS:
1. Select 1 Child: $80 per month, or 2 Children: $120 per month, or 3 Children: $150 per month
2. Send appropriate payment amount to: SoCalKenpoKarate@gmail.com
3. IMPORTANT: PLEASE DESIGNATE PAYMENT TYPE AS "SEND TO A FRIEND"
AND NOT "GOODS OR SERVICE" TO AVOID ADDITIONAL FEES.
